The submission of this application is a required step for all candidates seeking enrollment in the Ponytail Academy Advanced Course.
However, submission is not a guarantee of selection or approval to attend the course.

All applications will be thoroughly reviewed and selection will be based on a comprehensive assessment of the applicant's qualifications, including:

• Demonstrated Experience: Relevant experience in the field.

• Prerequisite Completion: Successful completion of the Ponytail Academy Beginner Course (6 hours) and/or the Intermediate Course (12+ hours).

Only applicants who meet the necessary criteria will be contacted for final enrollment approval.

In order to maintain the integrity and proprietary rights of the Ponytail Lift and Ponytail Facelift trademarks, no attendee will be permitted to promote or perform these specific trademarked techniques unless formal, written permission is explicitly granted by Dr. Chia Chi Kao.

Failure to strictly abide by this intellectual property restriction will result in immediate legal action.
